Brooke Seebohm
Mental Health Nurse
Brooke is a Registered Nurse with a Master of Mental Health Nursing and 13 years of experience across public and private psychiatric services, specialising in emergency and acute psychiatry.
She works with people experiencing complex and enduring mental health concerns, providing ongoing therapeutic support, relapse prevention, early identification of changes in mental health and care coordination.
Brooke values developing long-term therapeutic relationships with clients and their families, providing personalised support that promotes stability, recovery and greater understanding of mental health. She works collaboratively with families and carers and, where appropriate, alongside psychiatrists, psychologists, GPs and other treating clinicians to support integrated care.
She also provides support with care coordination and liaison with public and private mental health services, as well as clinical consultation for health professionals seeking specialist mental health nursing input.
Brooke provides voluntary, community-based care and does not provide case management for people currently subject to the Mental Health Act. Her role is focused on supporting clients who are able to engage collaboratively in their care, while working alongside existing treating teams.
Self-referrals and professional referrals welcome.
